Integrated Services For Older People – EYPCT & YWCPCT<br />
This is an integrated service that works to prevent hospital admission <strong>and</strong><br />
to try <strong>and</strong> help people stay in their own homes.<br />
The team comprises <strong>of</strong> physiotherapists, occupational therapists,<br />
social services assessment <strong>of</strong>ficers, dietetics <strong>and</strong> Community Matrons.<br />
Referrals can only be made by a health pr<strong>of</strong>essional.<br />
Currently, the Community Matron service for the <strong>East</strong> <strong>Riding</strong> is only<br />
available for patients with a GP in the <strong>East</strong> <strong>Riding</strong> <strong>of</strong> <strong>Yorkshire</strong> area.<br />

NCH Community Transport Project<br />
NCH Community Transport Project, in partnership with the Children’s Fund,<br />
